Previous research has focused on the effects of various risk communication formats, but less is known about which communication formats are best suited for patients to understand and recall risk information. We aimed to (1) understand why patients struggle with understanding and remembering risk information, (2) what communication formats facilitate patient understanding and recall of risk information, and (3) how effective patients believe various communication formats to be. In a two-phase qualitative study, 50 adult patients with a history of acute coronary syndrome (ACS) were interviewed about their understanding of and recall of risk information after attending a nurse-led education programme. Using content analysis, they were divided into three groups, based on how they initially interpreted and then recalled the risk information. Each participant then rated how effective each communication format was. Most patients initially misunderstood the risk information and subsequently forgot it. The communication format with which they initially interpreted and recalled the risk information were different and dependent on patient group. Some communication formats were perceived to be more effective than others. This study provides valuable insights into why patients struggle to understand and remember risk information, and a basis for informing the selection of effective communication formats.
